Anticoagulation service
What is this service?
Anticoagulation is an established treatment for patients at high risk of thromboembolism ( blood clots), either following an episode of venous thromboembolism (VTE), or, for those with Atrial Fibrillation (AF) or prosthetic heart valves.
Our 11 member GP practices provide the anticoagulation service in York. This means people on warfarin can be seen locally by fully trained and experienced staff, rather than going to hospital for their blood test.
Appropriate anticoagulation therapy (adjusted-dose warfarin) in people with AF can reduce mortality and morbidity

Background
Previously the monitoring of anticoagulation for patients taking oral anticoagulants has been preformed in a hospital clinical setting. This activity has now been transferred into primary care.
It has been shown that anticoagulation control by GP Practices, is as good as in hospital and is more convenient for patients. It is quicker and safer and is a more efficient way of carrying out this care.
As an ‘at scale’ provider, we're responsible for the monitoring and management of this service.
